Collaboration with primary care, the residential sector and with mental health
Cooperation with primary care providers
Cooperation with primary care providersThe hospital works very closely with the primary care organisations, such as family assistance, health insurance schemes, Centres for General Welfare, Public Centres for Social Welfare, home healthcare, paramedical services, etc.
The hospital maintains a neutral position with the selection of which organisation will provide follow-up care. The patient makes the selection, either with or without consulting the GP.
The hospital does work in close collaboration with specialised home nursing teams from the White and Yellow Cross of East Flanders and I-Mens.
In addition, the hospital is also involved in the following primary care zones:
- Primary care zone Schelde-Leie:
- Primary care zone Scheldekracht:
- Primary care zone Ghent
- Primary care zone Meetjesland East and West Flanders
- Primary care zone Flemish Ardennes
- Primary care zone Panacea
The objective of such cooperative efforts is to offer care that is better integrated, which also facilitates continuity of care before and after the hospital admission.

Please consult the following organisations and website for more information:
The PAKT is the mental healthcare network for adults (Article 107) in the Ghent-Eeklo-Flemish Ardennes region. The network partners are connected by a common goal of (re)organising care so as to provide every resident in the working area access to the most appropriate form of care, either upon request or need. They achieve this by implementing strategies based on the regional mental healthcare needs.
See the PAKT website to review initiatives within the regional mental healthcare system for adults, as well as the mental healthcare offerings within the region.
RADAR is the network for paediatric and adolescent mental healthcare in East Flanders. It is a collaboration between departments and organisations from various sectors that work on the mental health of children and adolescents within their environment in East Flanders. RADAR works with and for children and adolescents up to the age of 18, and with consideration for adolescents/young adults up to the age of 25 years old.
On the RADAR website, you may review the initiatives within the regional mental healthcare system for adults, as well as the mental healthcare offerings within the region.
The Flemish Discussion Platform for Mental Health connects individuals and organisations who are active in the domain of mental health, and supports them in various ways in the work they do. The Flemish Discussion Platform for Mental Health is recognised and subsidised by the Flemish governmental Agency for Care and Health.
The Flemish Discussion Platform for Mental Health strives to improve mental health for everyone, and works with all involved parties to provide high-quality and goal-oriented mental healthcare.
They are focused on organisations, care providers, patients and family members from mental healthcare as well as from other sectors that are closely related or involved with the goals of achieving improved mental healthcare for all.
