Preparing for your admission

In case of a planned admission of one or more days to our hospital, it is best to discuss and arrange a number of things in advance. This way, if necessary, preliminary examinations can still be carried out on time and the doctors concerned will have the results available in good time. Our pre-admission service will guide you through this and complete your admission file.

What does the pre-admission service do?

Our pre-admission team works with you to make the necessary administrative, informative and nursing preparations for your admission:

• Administrative: all administration associated with admission is put in order.
• Informative: you receive information on the course of the admission and the guidelines to be followed if you undergo surgery. You will receive a welcome pack containing various information brochures.
• Nursing: the nurse will take stock of your medical history and current state of health based on the data you have collected. Where necessary, additional preoperative examinations will be requested by the anaesthetist. At the latest 48 hours before the start of surgery, he will review your preoperative file. If the results do not meet expectations, your GP may be contacted or additional tests may be requested (e.g. MRSA).

Contact with our pre-operation team is made via the pre-operation desk. In AZ Maria Middelares, this can also be done digitally via the digital patient platform MijnZiekenhuis.

Administrative

• Identity card
• Name, address and telephone number of a contact person to whom we can turn if necessary during your stay
• Proof of hospitalisation insurance (Assurcard, DKV card)
• Insurance forms in case of occupational accident (policy)
• Guarantee declaration of foreign insurance
• Your registration card if you call on the CPAS

Medical

Be sure to bring the following items (if applicable):

• Documents provided by your attending physician before admission
• Refund certificates of medicines
• Prescriptions for allergies or your diet

Informed consent

If applicable, complete one of these documents and return it signed no later than the day of your surgery.

Medicines

Your home medication may be replaced by an identical medicine that the hospital stocks. The medicine may have a different colour or shape, but has the same effect. If you still need to take this medication when you return home, you may switch back to your familiar medication. On discharge, your doctor will give you clear guidelines on which home medication you should and should not continue to take.

It is important that your carers know which medicines you were taking at home (or shortly before your admission).

In the admission information pack you will find the following tools.

1. Medication overview list / home medication: make sure it is filled in completely and correctly. Ask your GP or pharmacist for help if necessary.
2. Paper medication bag (in AZ Sint-Vincentius Deinze): put your medicines in their original packaging in this bag and bring it with you to the hospital.

On arrival at the ward, a nurse will go over the form with you. If you are allergic to certain medicines, please mention this during this conversation.

The home medication you bring with you will be kept in the nursing station. You will get them back on discharge from hospital.

During your stay, you will receive medicines from the hospital. Use your own medicines only in consultation with your healthcare provider. After all, certain combinations may have undesirable effects.

Personal

• Toiletries: towels, flannels, soap, toothbrush, toothpaste, comb, shaving kit
• (Sleeping) clothes, dressing gown and closed shoes
• Sturdy sports shoes if you receive rehabilitation
• Walking aids such as crutches, walker, cane ... marked with your name and surname

Are you coming for an operation or examination? Limit the luggage you bring to the bare essentials. For this, use a bag or suitcase with the usual dimensions of hand luggage: maximum 55x20x40 cm. This way, you can be sure that your belongings can be safely stored in a locker.

It is better to leave large sums of money or valuable items at home. The hospital is not liable for loss or theft. Jewellery and other jewellery (e.g. piercings) are not allowed on admission for hygienic reasons. Leave these at home.

• AZ Maria Middelares: do you have anything valuable with you? Name any possessions. You can store your valuables in a safe in your room. If it does not fit in the safe, we can deposit it in the hospital safe. On discharge, you can collect everything again.
• AZ Sint-Vincentius Deinze: in an emergency, you can use the hospital safe to store valuable items during your admission.

You can choose from the following room types:

• quadruple room
• double room
• single room

The choice of room does not affect the quality of care. Please understand if, due to lack of space, we cannot immediately give you the room you requested.

The price of the room (nursing day price) is paid by the NIHDI. The co-payments and additional hotel costs (the personal share) remain the responsibility of the patient.

When staying in a single room, you will be charged a supplement. With this choice of room, doctors' fees may be increased. In the admissions department there is a list available for inspection with the maximum fee supplements per doctor. Read more about the price of a room and fee supplements here.

On the day of your (pre)admission, you will sign an admission statement. In it you will find an overview of your personal contributions to the costs. Our staff will go over the admission declaration with you. By signing these documents, you confirm that you agree with

• your choice of room;
• the rates and your personal contributions;
• the financial conditions of your admission.

Are you admitted to AZ Maria Middelares or AZ Sint-Vincentius Deinze for an operation or examination with a short stay? Then it is possible that you will be taken to the discharge lounge soon after your operation or examination, allowing you to go home sooner. If this is possible for you, your doctor will discuss this with you beforehand.
