For a copy of your medical record, please contact:

|On www.cozo.be or via their smartphone app. You can view your data (reports, results, images, medication schedules, letters ...) digitally on this digital cooperation platform. The platform contains your data as of 1 December 2017. If you consulted a physician at Maria Middelares General Hospital (Ghent location) or the Maria Middelares Medical Centre (Ghent-Bruges), you can view your data digitally as of three weeks after the visit. This period of time allows the physician to first discuss the results with you and to provide you with the necessary explanations.|

Confidential contact person or representativeConfidential contact person or representative
If you wish to appoint a confidential contact person or a representative regarding a copy or viewing your records, please refer to the respective authorisation forms of the Federal Public Health Service:
- Representative (You would like to name a representative):
If the patient is incapable of exercising their patient rights, the representative is the person who exercises the patient’s rights in their name. This differentiates them from the confidential contact person who only provides the patient with support.
- Confidential advisor (You wish to appoint a confidential contact person):
a family member, a friend or another patient or any other person who is appointed by the patient to assist them in obtaining information about their state of health, in viewing their medical record or in obtaining a copy thereof, as well as in submitting a complaint.
